Professional callback forms collect the dentist’s name, phone number, professional email, practice name, GDC number, broad referral area and preferred callback timing. We also record the page used and submission time so the team can route and audit the enquiry.

Please keep clinical details out of website forms

Do not enter medical history, diagnoses, patient names, radiographs or clinical records into these website forms. A broad dental concern can reveal limited health information, so Smile2Life uses it only to route the requested contact. The team will explain the appropriate secure process for any clinical information that is needed later.

How information is used and stored

Your submission is sent securely to the website service, stored in its protected enquiry database and passed to Smile2Life’s Wix CRM so the team can respond, arrange the appropriate next step, keep an enquiry record and resolve delivery issues. Access is restricted to authorised people and service providers supporting the website and CRM. Smile2Life does not sell enquiry information or use it for marketing without separate permission.
